SUBMIT A CLAIM

To submit a claim on your Brock Health Private Health Services Plan (PHSP), complete the following steps:

1. Go to the Forms page and download a claim form. There are versions for provinces with GST and versions for provinces with HST. There are also spreadsheet versions using Microsoft Excel that save you the arithmetic if you have access to that software.

2. You can either print the form and complete it or put your information into the spreadsheet and print it. The receipts must be enclosed with the claim form. They do not have to be in any order.

3. If you are not the Planholder (the employer). Then simply deliver your claim to your employer who will make the submission on your behalf. Expect payment to arrive at your address via Canada Post within 10 business days of the date your employer makes your submission.

We also offer Direct Deposit for no charge. If you want to sign up for this service, with your next claim, simply enclose a void cheque for the personal account where you want the funds deposited. We can not make Direct Deposits to business accounts; it must be the personal accounts of the employee.

The exception is the case of sole proprietors who use a single account for business and personal funds. Once you are signed up we will continue to deposit reimbursement cheques to that account until you notify us otherwise. Direct Deposit payments are made within 3 business days of the receipt of your claim.

4. The Planholder will enclose a claim payment cheque made payable to Brock Health Administration drawn on the business bank account in Canada for the Total Fee Payable (Health Expenses + 5% Administration Fee + GST/HST).

5. Planholder mails the forms and cheque to:

We are committed to responding to your submission within 5 business days of receiving your claim. For reimbursements sent by cheque, depending on the distance and your Canada Post delivery cycle, your refund cheque generally arrives within 10 business days. For Direct Deposit reimbursements, the payments are in your account within 3 business days. If you provide a contact email address we will send email notifications of receipt of your claim and dispatch of your cheque.
