HOW TO ENROLL IN A PHSP

Setting up a Private Health Services Plan (PHSP) with Brock Health can be done in two ways. We offer online enrolment if you wish to use a credit card for payment, or if you want to pay by cheque you can mail in the enrolment form through Canada Post.

Option 1 - Online Enrolment

Brock Health Administration Ltd. has selected a Canadian company Tooq Inc. for processing our online enrolments. We were convinced by the integrity of their security and privacy commitments to online personal and financial data. A key factor in this choice was their use of publicly traded, British Columbia based Beanstream.com for payment processing. Unlike the larger and more commonly known Internet payment companies, primarily American, Beanstream meets security standards of the international Payment Card Industry. (PCI compliant)

At Brock Health, we know our customers expect robust systems to protect their privacy. We are proud to place our confidence in Tooq Inc. to extend that expectation to online enrolments. By clicking on this link you will be sent to their secure site where your online Brock Health PHSP enrolment and credit card payment will be processed. On completion you will be returned to Brock Health website.

Option 2 - Mail-In Enrolment

The postal mail enrolment method is a simple 4 step process:

1. Download the Enrolment Form. If you have additional employees other than yourself you should also download Appendix A. Click on these links to download the forms.

2. Print off and fill out the form (s). Be sure to sign it. This is your 3rd Party Contract of health benefit coverage. If required, use Appendix A to list the names of your Covered Employees.

3. Enclose a cheque drawn on your business bank account in Canada for the $100 Enrolment Fee made payable to Brock Health Administration.

4. Planholder mails the forms and cheque to:

You can also submit your first claim along with your enrolment application. We will open your account and adjudicate your claim at the same time. We are committed to responding with your contract and payment within 5 business days of receiving your claim. You can download a Claim Form here.
